Most dental problems are resolved without surgery. Surgery becomes the appropriate route when a tooth is fractured below the gum line, decayed beyond predictable restoration, infected at the root with no reliable path to repair, impacted beneath bone or gum, or when removing it is a necessary step in a larger plan such as implant treatment or orthodontics. The judgement is clinical rather than automatic: the same broken tooth may be restorable in one mouth and not in another, depending on how much sound structure remains, the root anatomy, the condition of the surrounding bone and what the final result needs to be. You should expect that reasoning to be explained to you, along with the alternatives where alternatives exist, before any decision is made.
Assessment begins with examination and radiographs, and where the anatomy needs to be seen in three dimensions, a CBCT scan. That matters most when a lower root sits close to the inferior alveolar nerve, when an upper root is close to the sinus floor, or when a tooth is impacted and its true position cannot be judged from a flat image. Your medical history is reviewed in the same appointment, because it changes how surgery is planned: blood-thinning medication, bisphosphonates and related bone medications, diabetes, immune conditions and previous radiotherapy to the head or neck all influence timing, technique and healing. Bringing a current list of your medications to the consultation is one of the most useful things you can arrive with.
Most oral surgery at Apple Dental Specialists is carried out in our own clinic under local anaesthetic, with sedation added where it suits the procedure or the patient. The site is numbed thoroughly and tested before anything begins. Depending on what is being treated, a tooth may be removed whole or sectioned into pieces so it can be released without unnecessary force on the surrounding bone, and a small amount of bone may be reshaped to allow access. Sutures are placed where the site heals better closed. Where an implant is planned for the same site later, the socket may be grafted at the time of removal to preserve bone that would otherwise resorb. You leave with written aftercare instructions and a number to call.
Local anaesthetic makes the procedure itself painless. Sedation addresses everything around it — the anticipation, the sounds, the length of the appointment and the memory of it afterwards. For many extractions, local anaesthetic alone is entirely sufficient. For longer surgical appointments, for impacted teeth, for patients with a strong gag reflex, and for anyone whose anxiety has kept them out of a dental chair for years, IV sedation with a specialist anesthesiologist can be arranged. Sedation is a separate decision with its own assessment, its own consent and its own fee, discussed and quoted in advance. If you are unsure whether you need it, raise it at the consultation — it is a common question, not an unusual one.
Swelling usually peaks around 48 to 72 hours after surgery and then settles. Bruising, jaw stiffness and tenderness for several days are normal rather than a sign that something has gone wrong. Most people manage with the pain relief advised for them and return to desk-based work within a day or two, while heavy physical work and exercise are usually delayed a little longer. The instructions that matter most in the first 24 hours are the dull ones: leave the clot undisturbed, avoid vigorous rinsing, avoid smoking, and do not drink through a straw. A single-visit extraction of a fully erupted tooth and a multi-stage surgical procedure are both oral surgery, which is why one figure cannot describe it. The range reflects that spread. Your own figure is confirmed in writing after examination and imaging, before treatment is booked.
A fully erupted tooth with simple root anatomy is a different procedure from one that is impacted beneath bone, curved or splayed at the root, or fused to the surrounding bone.
Socket preservation or grafting at the time of removal adds cost at that appointment, but it protects bone volume for a future implant and is usually less involved than rebuilding the site later.
Sedation is assessed, consented and quoted separately from the surgery itself, and it is confirmed with you in advance rather than raised at the appointment.
A CBCT scan is used where the relationship between a root and the nerve or the sinus needs to be seen in three dimensions rather than estimated from a two-dimensional radiograph.
